Telephone Calls: You may leave a message anytime at 1-440-442-8600. I will return your call as soon as possible. Please note that counseling is most effective face-to-face. If necessary, telephone consultation can be provided at the same rate as an office visit. If an emergency occurs and you cannot reach me, call 911 or go to the nearest emergency room.

Cancellations: Please schedule appointment times that you are reasonably certain you can keep. If you must cancel an appointment, please do so at least 24 hours in advance. If you handle a cancellation in this manner, it is possible to offer your scheduled time to another person who needs it. Cancellations made without this 24-hour advance notice may be charged to you. Be advised that your insurance company cannot be billed for last-minute cancellations; therefore, the entire cost of that session may be your responsibility.

Insurance: As a courtesy, this office will submit insurance claims to your insurance carrier on your behalf. However, your insurance contract is between you and your insurance company. Ultimately, you are responsible for payment for services. In accepting this courtesy, you agree not to hold this office liable for insurance processing problems.

Fees: The current fee is $150 per clinical hour (45-50) minutes. Unless otherwise specified, any session longer than that will be charged at the hourly rate.

Payments: It is simplest if payment (or copayment) is made at the time of service. Your billing statements will reflect services given and payments received as well as the usual, customary, and reasonable (UCR) adjustments applied. Balances that remain unpaid for an unreasonable length of time may be subject to a 1.5% per month interest charge. If you can’t pay the balance, do not hesitate to call me to arrange a payment plan.

Information Update: Please notify me of any changes in your address, telephone number or insurance information.

Termination: At the conclusion of treatment, a “closing session” is recommended in order to examine what led to engaging in the therapeutic process, what techniques you have learned your progress and the basis on which any future service may be helpful.
